Don't take spotting as the start of your period. Take the first day of heavy menstrual bleeding as the first day of menstruation; if your period starts after 8 pm, take the next day as the first day of menstruation.

The injections are often started on the 2nd or 3rd day of the cycle (the 2nd or 3rd day of menstrual bleeding). In the case of another type of protocol (called a long protocol), the start of the injections is precisely marked in the treatment plan you received at our clinic.

Stimulant medications requiring dilution i.e. containing the powder itself and dilution water can be stored in a dry place at room temperature, the same way Orgalutran and Cetrotide are stored. It is recommended to keep medicines administered with the Gonal and Puregon stimulation pen in the refrigerator (2-8 °C). In addition, the ovulation induction drug Pregnyl should be stored in the refrigerator; the other ovulation induction drug Ovitrelle can be stored at room temperature up to 25 °C.

You can travel with your stimulation medication without any problems, and medications that need to be kept in the refrigerator will keep at room temperature for up to 3 months (Puregon) and 1 month (Gonal). Never leave medicines in the car.

The most important thing is to apply the medication regularly at approximately the same time every day. It is up to you whether you choose morning or evening.

If you break a vial, spill its contents, or are unsure of the correct application, please call our hotline. Your doctor will advise you on how to proceed. It is essential to maintain continuity of administration.

Each pen has a reserve of around 75 to 100 units, so it is perfectly fine if you have some left in the pen. You can use this "reserve" in the continuation of the stimulation.

První ultrazvukové vyšetření se provádí nejčastěji 6. - 7.den stimulace, další UZ vyšetření je obvykle za 2 - 3 dny poté. Na první vyšetření se telefonicky objednejte nejbližší pracovní den (8 - 15 hod) po zahájení stimulace. Provádí-li se UZ vyšetření v místě Vašeho bydliště, oznamte emailem zahájení stimulace a datum kontroly Vašemu ošetřujícímu lékaři na naší klinice.

V případě, že jste mladší než 40 let, není anesteziologem vyžadováno interní předoperační vyšetření, a proto není potřeba brát ani žádné odběry. V den odběru vajíček bude Vám i Vašemu partnerovi odebrána krev na vyšetření některých předepsaných infekcí. Toto vyšetření je nezbytné, předepsané zákonem.

Hodina aplikace Pregnylu je zásadní pro časování odběru vajíček (odběr by měl proběhnout 35 – 37 hodin po aplikaci Pregnylu), proto se ji prosím pokuste dodržet co nepřesněji. V případě, že zapomenete Pregnyl aplikovat, zavolejte prosím na naši nonstop linku a domluvte se sloužícím lékařem na dalším postupu.Bezhlavé dodatečné aplikování Pregnylu může přinést komplikaci s časovým naplánováním odběru. V případě, že Pregnyl neaplikujete vůbec nebo špatně, při odběru nebude získáno žádné (!) vajíčko.

Do kožní řasy na břiše, stejně tak jako jste aplikovala stimulační injekce FSH. Intramuskulární aplikace, tak jak je napsáno v návodu je zbytečně bolestivá a nepřináší lepší efekt na dozrání vajíček.

Doporučujeme 2 dny pohlavní abstinence, zejména proto, aby byl dostatečný objem ejakulátu pro zpracování laboratoři. Na druhou stranu není potřeba dlouhodobé abstinence, protože to může mít negativní vliv na morfologii a pohyblivost spermií.

The quality of sperm is influenced by all factors that the man has been in contact with over the last 3 months before collection. Therefore, there is no need for any special regime shortly before the collection of ejaculate.

K odběru vajíček přijděte na lačno (od půlnoci nejíst, nepít a nekouřit), s partnerem, s sebou si vezměte delší triko nebo noční košili, přezůvky a intimní vložku – po odběru můžete slabě krvácet.

Samotný zákrok probíhá v celkové anestézii, není potřeba se ho obávat. Délka výkonu závisí na počtu vajíček a obvykle nepřesahuje 10 minut. Bolestivost po zákroku je vnímána individuálně a lze ji tlumit analgetiky. Za cca 2 hodiny po zákroku budete lékařem propuštěna domů. Po anestézii nesmíte řídit.

Po odběru vajíček, při konzultaci s lékařem, budou definitivně upřesněny všechny procedury, které budou ve Vašem případě provedeny a celý plán si společně odsouhlasíte. Zároveň Vám lékař sdělí předběžnou částku, kterou budete platit. Konečná částka by se od této předběžné neměla již zásadně lišit. Platit po punkci budete v případě, že budete mít plánované genetické testování nebo nebudete mít čerstvý transfer.

Oocyte donation is bilaterally anonymous.

The method of selecting donors is highly sophisticated. The selection takes place well in advance with the aim to meet as many of your requirements as possible. All donors must have a full 12-year education and they must be citizens of the Czech Republic. At our clinic we only use eggs from young healthy donors – women under 32. To enter the donation programme the donor must meet very strict criteria.

Your photo is not necessary for the selection of a donor.

The suitable donor is selected 6 weeks prior to oocyte retrieval.

Information about the donor (such as age and blood group) will be provided 2 to 3 weeks before oocyte retrieval.

The suitable donor must have negative results for sexually transmitted diseases, we also examine the karyotype and cystic fibrosis.

The treatment can be performed until 49th birthday of the recipient.

Women over 45 are recommended to undergo mammography and they need a certificate from their gynaecologist that they can withstand pregnancy.

You and your partner will need a STD test statement (sexually transmitted diseases) not older than 6 months.
Your partner will need the results of his sperm count analysis.

We recommend 2 days of sexual abstinence, especially to ensure a sufficient volume of ejaculate for laboratory processing. On the other hand it is not necessary to resort to long-term abstinence, because it may negatively impact the morphology and motility of the sperm.

Sperm quality is a result of multiple factors that influenced the given man over the last three months before sampling. Therefore no special regime is required during the short period before ejaculate sampling.

Light bleeding or spotting after egg retrieval is common. It is caused by needle penetration through the vagina during ovarian puncture. If the bleeding is heavy or persists the next day, call our hotline and your doctor will advise you on how to proceed.

You may feel a slight pain in your lower abdomen on the day of the puncture and the following day. This is due to the enlargement of the ovaries by stimulant drugs. Worsening pain on the second or third day after the puncture may indicate the development of hyperstimulation, so call our hotline instead. Your doctor will most likely recommend a personal visit to the clinic.

You will be given a medication schedule in a timely manner by your coordinator or doctor. It usually consists of progesterone and estrogens to support the uterine lining. Taking other medications is always only after consulting your doctor.

The day of the embryo transfer is planned by the doctor in collaboration with the embryologist and the patient. The transfer is performed on the 3rd or 5th day after egg retrieval or donor puncture. The coordinator will inform you about the date and time of the embryo transfer.

The high success rate of the treatment allows us to transfer one embryo. We recommend freezing the remaining good-quality embryos.

The quality of embryos can be assessed from the second day of their development. Two-day-old embryo should consist of 3-4 cells. Three-day-old embryo has 6-8 cells. On the fourth day the embryo becomes a morula and it is very hard to assess its quality. On the 5th day the embryo becomes a blastocyst.

You do not need to bring anything special for embryotransfer. Bring all your signed consents with your treatment and also signed consent with freezing of embryos, if you wish to preserve the surplus embryos by freezing.

You do not need to come with extremely full bladder for embryotransfer. We just recommend not emptying your bladder immediately before the embryotransfer. Half-full bladder helps to keep the uterus in an upright position and facilitates the transfer of embryos into the uterus.

Embryo transfer usually takes 3-5 minutes. After the transfer, you will remain lying in the transfer room for about 10 - 15 minutes before you can go home.

Presence of your partner during embryotransfer is a standard (unless you wish otherwise).

IVF with your eggs: you will be informed of the preliminary price for the treatment before the stimulation begins. On the day of egg retrieval, you will be informed again about the proposed treatments and their cost. Final payment will then be made on the day of the fresh embryo transfer. At the clinic, you can pay in cash or by credit card (in CZK and EUR). In case the fresh embryo transfer is not part of the scheduled treatment (e.g. during a scheduled PGT or for other reasons...), you will need to pay the deposit for the treatment on the day of egg retrieval. An invoice with the remaining amount will be emailed to you within a week after the embryos have been frozen. The due date is 1 week.

IVF with donated eggs: the patient is required to make a deposit of 2500 EUR when the specific day for the egg retrieval procedure of the donor is confirmed.

After embryotransfer you do not need to lay still. We recommend the usual daily routine with elimination of stress factors such as physically challenging sports or hard manual work. Sick leave from work is not necessary.

After embryotransfer you do not have withdraw from trave. Our patients usually travel home on the day of the embryotransfer, or the day after. You can use any means of transport for your journey home.

Embryos nest (nidulate) in the uterine mucosa usually on the 6th - 8th day after fertilisation, which is 24 - 48 hours following embryotransfer.

You may have a light bloody show after embryotransfer but it should not last more than one day. It is caused by slight bruising of the uterine cervix during transfer. This show does not impact the result of your treatment. Heavier or longer bleeding shall be consulted with your physician.

Continue using all the medication that you had used before embryotransfer - it is always at least progesteron. If you also had used Estrimax, Anopyrin or any other medication before embryotransfer, continue using them further.

If you swallowed progesteron, start inserting it into the vagina (in the same dose) twice per day (in the morning and evening). If you are inserting it already, you can switch to progesteron gel.

Do the pregnancy test after two weeks following embryotransfer; the exact date is stated in the final report.

In case of negative test from urine you can undergo pregnancy testing from blood just to be sure. If the negative result is confirmed stop using all the medications relating to your treatment at our clinic.

Please continue using all your medications. If it was a pregnancy test from urine, you can confirm the result by blood test, but it is not necessary. Book for ultrasound examination after 14 days following the positive test.

Do not stop using any of the medications, plenty of resting is recommended. Increase your progesteron dose by two pills in the midday. In case that you have a bloody show before the term of your pregnancy test, wait and take the test on the given day. In case of already confirmed positive result please book for blood screening after three days following the test.

Ultrasound screening shall be performed after 3 - 4 weeks following embryotransfer.

The length of pregnancy (number of weeks and days of your pregnancy) in natural cycle is not counted from the date of fertilisation, but always from the first day of your last period. This method is applied also in cycles of assisted reproduction, although we know the exact date of conception.To calculate the approximate length of your pregnancy use the day of embryotransfer as day 19. This means that on the day of transfer you have been pregnant for 2 weeks and 5 days.

Use all the medicaments without any changes (unless the physician told you otherwise before the transfer) until the first ultrasound screening , which shall be planned after 14 days following the positive pregnancy test.

We recommend using all the medicaments without any change until the end of the 8th week of pregnancy. Of course every patient is using different medications and getting off them is always individual. In general we can say that you can quit immunosuppressive drugs after the ultrasound proof of foetal heart action. Dose of oestrogens and progesteron can be reduced to half after week 8 and stopped completely at the begining of week 10. Accurate timing of getting off medicaments after successful IVF cycle must be always consulted with your physician.

All tests from urine are not equally sensitive, please repeat the test with a different type of urine strip after 2 days, and if it is still negative and you start to bleed, it means your treatment was not successful. If the test is negative and you are not bleeding, or have just light bloody show, it is necessary to test again from blood.

If you had repeatedly negative pregnancy tests, you can get off all the medications at once. The only exception are immunosuppressive drugs in a dose over 10 mg (2tbl) per day. Consult your prescribing physician about getting off immunosuppressives gradually.

If you have frozen embryos from a previous cycle with donor eggs you can, plan cryoembryotransfer for the next following cycle.If you need a new treatment with donor oocytes, you can perform it after 3-4 months. Please consult your physician about the next steps.
