Starting a family after forty
Have you been dreaming of starting a family after you turn 40? We can help you increase your odds with a customised treatment plan.
As we all know, it can be challenging to get pregnant and give birth to a healthy baby as we age. We really understand this, and we're here to help! We include procedures in our treatment programmes that we know will significantly increase your chances of starting a family.

What can increase your chances of starting a family
Individualised stimulation to obtain high-quality eggs
A woman's fertility naturally starts to decline around the age of 30. The main reasons are the declining quantity and quality of eggs. There are also other reasons such as associated diseases. So, choosing the best stimulation type for you is very important. Our goal is to obtain sufficient quality eggs suitable for fertilisation.

Chances of natural conception decline after 40
Once you reach the age of 40, your chance of conceiving naturally in a single cycle is about 5%. By 40, the original pool of 2 million eggs is reduced to about 5,000.
Advantages
- Without hormone stimulation
- Lower cost - see the price list
Disadantages
- Relatively low success rate compared to standard IVF because only one egg is retrieved
Suitability of the procedure
- Low egg reserve
- Fear of or inability to take hormones (e.g. after breast cancer treatment)
Advantages
- With minimal hormone stimulation
- Lower cost - see the price
Disadvantages
- Lower success rate than standard IVF because fewer eggs are retrieved
Suitability of the procedure
- Low egg reserve
- Repeated failure of gonadotropin stimulation during an IVF programme
- Risk of developing ovarian hyperstimulation syndrome
Advantages
- Maximising oocyte yield
- Low risk of developing Ovarian Hyperstimulation Syndrome
Disadvantages
- Risk of developing Ovarian Hyperstimulation Syndrome with long protocol treatment
- Higher cost
Suitability of the procedure
- Previous stimulations were unsuccessful/resulted in poor-quality eggs or embryos

The quality of own eggs decreases with age
As shown in the graph, the quantity and quality of eggs decreases dramatically after the age of 37.
The probability of getting pregnant with your eggs is about 1% by the age of 45.

IVF with donated eggs - higher chances of success
We, therefore, recommend that you consider using donor eggs in these cases. Your age does not affect the chances of a successful pregnancy, as the age of the uterus does not play a significant role. At Reprofit, we have an extensive database of anonymous, young and healthy donors from the Czech and Slovak Republics (i.e. donated eggs are NOT obtained from women undergoing fertility treatment).
Selection of the best-quality sperm
If a sperm with damaged DNA fertilises an egg, the embryo may stop developing or develop genetic defects or later miscarry.
Unlike a woman, a man's sperm is renewed approximately every three months. However, there is an increasing number of cases where men have slowed or poor-quality sperm that are less able to conceive. There is also a rapid increase in various abnormalities, which can be genetic but also acquired during life. The quality of sperm begins to decline gradually from the age of 40.
Therefore, we offer high-quality and thorough sperm testing according to World Health Organization recommendations and complementary methods to select the best quality sperm to fertilise the egg. This increases the chances of a healthy embryo and successful treatment.

Unexplained infertility
In men with otherwise good spermiogram, a higher degree of DNA damage can cause unexplained infertility.
Advantages
- Proven to reduce the amount of sperms with damaged DNA
- Increases the proportion of genetically healthy embryos in some instances
- Increases overall cycle success rate by up to 35%
- It does not require prior treatment of the semen sample or the use of devices that may, in some cases, increase oxidative stress in spermatozoa
Suitability of the procedure
- When embryo development has been impaired in previous IVF cycles
- After repeated embryo transfers without subsequent embryo implantation
- In recurrent miscarriages
- In cases of repeated insemination failure
- When a low number of oocytes is obtained from the woman
Price
340 EUR
Advantages
- Allows selection of mature sperm with intact DNA
- Reduces the incidence of chromosomal abnormalities in sperm DNA
- Increases the rate of egg fertilisation and healthy embryo development by obtaining mature spermatozoa
- Selection utilises sperm binding to hyaluronan, a natural part of the Zona Pellucida
Suitability of the procedure
- For poor embryo development in previous IVF cycles
- In case of repeated embryo nesting failure
- In recurrent miscarriages
- In cases of reduced sperm motility
- When the number of eggs retrieved from the woman is low

Ensuring the best possible conditions for embryo development
After fertilisation, we place the embryos in a special incubator in our laboratory. Our experienced embryologists closely monitor their development. As a standard procedure, they briefly remove the embryos from the incubator and examine them under a microscope.
If you want to give your embryos the best possible conditions for their development, you can choose modern time-lapse incubators. They allow you to continuously monitor the development of the embryos without having to remove them. As a result, the embryo is not exposed to external influences and mechanical manipulation. The EmbryoScope+ incubator is also equipped with artificial intelligence to help the embryologist decide which embryo is best to transfer to the uterus or freeze for future use.

Embryocsope+ with Time-lapse
EmbryoScope+ with time-lapse. An advanced incubator with continuous embryo monitoring and AI support to ensure the best possible conditions for embryo development and selection.
Advantages
- Allows embryos to be monitored without removal, minimising potential exposure to unwanted external influences
- Allows detection of subtle changes in embryo development that may not be apparent with regular evaluation at only a certain time
- Continuous monitoring and high-quality imaging allow a more thorough analysis of embryo development, which helps to select the most viable embryos for transfer to the uterus

Selection of the best embryo
Egg quality is one of the most critical factors determining the quality of the embryo. At 40, a woman has about 40% of genetically normal eggs; four years later, only 10-20%.
We use prolonged cultivation to select the best embryo, which we consider standard at Reprofit. We only transfer the embryo on the fifth day after fertilisation, i.e. at the blastocyst stage, because only a viable embryo reaches this stage.
However, the eggs' quality significantly affects the embryos' quality. Unfortunately, older eggs have a significantly increased risk of chromosomal defects in the embryo, which will only become apparent during embryo development. However, these chromosomal defects are not visible to the naked eye. This is why we also recommend preimplantation genetic testing PGT-A for aneuploidy. This allows us to detect chromosomal defects in the embryo before transferring it to the uterus. Selected embryos have a significantly higher chance of uterine implantation and healthy development. In addition, using PGT-A substantially reduces the time needed to achieve a successful pregnancy.

Preimplantation genetic testing
PGT-A enables the identification of chromosomal abnormalities in the embryo, which can result in conditions such as Down's or Turner's syndrome.
Advantages
- It allows the detection of chromosomal defects in embryos and the selection of those suitable for implantation in the uterus
- Reduces the risk of miscarriage
- Significantly increases the success rate of infertility treatment

Facilitating the embryo's nesting in the uterus
Once the embryo has been transferred to the uterus, the next stage is nesting. Unfortunately, there are many reasons why even a good-quality embryo may fail to implant. One of these is a breakdown in communication between the embryo and the endometrium. This is where EmbryoGlue can help, as it promotes proper communication between the embryo tissue and the endometrium, increasing the chances of the embryo being retained after transfer.

EmbryoGlue
The utilisation of EmbryoGlue in embryo transfer procedures was observed to result in a 5% increase in the probability of successful pregnancy outcomes (as indicated by internal research findings).
Advantages
- Increases the likelihood that the embryo will attach successfully to the uterine wall
- Increases the success rate of IVF treatment
- Often helps when the cause of infertility is unknown

Using the latest scientific research
Our Science and Research Department has long been involved in several research projects on egg and embryo development in cooperation with renowned Czech and foreign institutions. The results of their research have been published, for example, in the prestigious scientific journal Nature Communication. We then translate these findings into our clinical practice.
Thanks to the cooperation with the scientific department, you can use the following methods:
• Stimulation of sperm movement (motility) - suitable, for example, for samples obtained after surgical sperm extraction.
• Examining egg maturation using polarisation microscopy (helps us to assess egg maturation more accurately and, in some cases, to optimise the appropriate time for fertilisation).
We follow global trends in assisted reproduction research. Currently, more attention is being paid to the following two procedures (due to the ambiguous results of various studies, they are still considered experimental and are not used in clinical practice in our country):
• Oocyte rejuvenation, based on the transfer of mitochondria, cytoplasm or nuclei from one egg to another;
• Improving embryo viability and chances of implantation by using enriched culture media.
